Prior to your appointment please fill in the online health questionairre and consent form to maximize treatment time.
We find that facts concerning your medical history are often best remembered when completing the following online form in the comfort of your own home.
FIRST APPOINTMENT
- Bring your insurance card
- Form of payment (debit or credit card)
- Please, arrive 10 minutes early to initial evaluation
On your first visit, your highly trained therapist will get to know you, gather information about your goals and the ability to perform the activities that are important to you.
Your therapist will then use your information to assess and develop a treatment plan for your specific goals. They will review this plan of care with you in detail so you have a complete understanding of your personalized treatment and recovery goals, answering any questions along the way.
With your plan in place, you’ll spend the last portion of your appointment actually beginning your treatment. Many patients experience improvement after their very first visit.
Your therapist will set a timeline for your recovery and teach home exercise program and lifestyle adjustment strategies critical to your healing and recovery process.
CANCELLATIONS & NO-SHOW
Keeping your treatment costs down and affordable is one of our goals.
Late cancellation and “no-shows” are very costly to healthcare providers.
These empty treatment slots also result in a missed opportunity to help heal other patients who often have life debilitating conditions.
A $90 no-show and late cancellation fee will be automatically charged.
To avoid this charge, cancellation notice must be provided
- via phone or voicemail only, AND
- with greater than 24 hours notice for Tuesday to Friday appointments, OR
- before 12pm Friday for Monday appointments.
This cancellation charge will not be covered by your insurance.
Extreme weather exceptions are in line with Boulder Valley School District’s snow cancellation framework (see here https://www.bvsd.org/ or google “BVSD snow day” for updates).
True verified sickness exceptions may apply and courtesy notice the night before is requested.
To keep costs down for all, we must strictly abide by this policy.
